Counterweight, Carrot, Crucible: Theorizing “Community” in Community Treatment Orders

Abstract

The idea of community in community mental health has recently been criticized as problematic in that it has been ill defined, under-investigated and under-theorized. In this article we introduce actor-network theory as a useful framework for the analysis of everyday psychiatric and legal interventions and their effects. We focus on the community treatment order (CTO) in Alberta as one such intervention, showing how community positions people diagnosed with mental illness as risky subjects. In examining the CTO as an important actor in the construction of mental illness, we argue that the idea of community acts as a counterweight to balance rights, a carrot in a mechanism of consent, and as a crucible where interpretive work on a subject must be done in chains of examinations. Further, we propose a procedural and relational definition of community to inform scholarly work as well as professional practice.

Résumé

Le concept de communauté dans le monde de la santé mentale a récemment été jugé problématique parce que mal défini, sous-étudié et sous-théorisé. Dans cet article, nous présentons la théorie de l’acteurréseau comme cadre utile pour l’analyse de la psychiatrie quotidienne et des interventions légales, de même que leurs effets. Nous nous intéressons en particulier à « l’ordonnance de traitement en milieu communautaire (OTMC) », plus précisément à un Community treatment order (CTO) en Alberta, afin de montrer jusqu’à quel point les personnes de la communauté diagnostiquées avec un trouble de santé mentale sont des personnes à risques. En considérant le CTO comme un acteur crucial dans le développement de la maladie mentale, nous avançons l’idée que les activités communautaires sont un contrepoids à l’équilibre des droits, un incitatif au mécanisme de consentement, et un milieu où le travail d’interprétation sur un sujet doit se faire selon un processus d’évaluation. Par ailleurs, nous proposons une définition formelle et relationnelle de la communauté aussi bien pour les travaux de recherche que pour la pratique professionnelle.
