Implementation of Action-Based Cognitive Remediation in First Episode Psychosis: A Multi-Method Evaluation of Feasibility and Efficacy

Abstract

We investigated the feasibility and effectiveness of an Action-Based Cognitive Remediation group within an outpatient early psychosis program. Sixteen participants completed the group including pre-post measures to assess quantitative outcomes of improvement. Eight participants completed individual, semistructured, qualitative interviews at 3–6 months post-intervention to assess feasibility and acceptability. The group was well tolerated. Significant improvements were observed in general cognition and functional capacity, which extended to psychiatric symptoms and illness stigma. Participants identified themes related to improved cognition and self-esteem, as well as generalizing skills to everyday activities. Action-Based Cognitive Remediation is a feasible and efficacious intervention for early psychosis.

Résumé

Nous avons étudié la faisabilité et l’efficacité d’un groupe de remédiation cognitive au sein d’un programme pour premiers épisodes psychotiques. Le groupe était formé de 16 participants et des mesures pré et postrencontres ont été prises afin d’évaluer les résultats quantitatifs en termes d’améliorations. Huit participants ont répondu à des interviews quantitatives individuelles semi-structurées entre 3 et 6 mois postintervention permettant ainsi d’évaluer la faisabilité et l’acceptabilité. Le groupe était bien accepté. Des améliorations significatives ont été observées en termes de cognition et de capacités fonctionnelles qui résultaient des symptômes psychiatriques et des préjugés sur la maladie. Les participants ont eux-mêmes déterminé des thèmes relatifs à l’amélioration de la cognition et de l’estime de soi, de même que des outils pour les activités quotidiennes. La remédiation cognitive est une intervention possible et efficace pour les premiers épisodes psychotiques.
