Adult Mental Health Sequels of Child Sexual Abuse, Physical Abuse and Neglect in Maternally Separated Children

Abstract

A follow-up study is reported of the mental health and psychosexual adjustment in young adulthood of 57 girls who were removed from home in childhood by Social Service agencies because of abuse, neglect or family breakdown. Twenty of the girls had experienced sexual abuse within the family context before the age of 14. The 57 maternally separated girls were compared with 30 girls who had experienced non-disrupted childhood. Multiple regression analysis showed that early sexual abuse explained more of the variance in adult adjustment than either physical abuse and neglect, or maternal separation. It is concluded that, subject to the limits of the sample, early sexual abuse within the family has severe long-term implications for mental health in adulthood unless appropriate therapeutic intervention is offered. Combinations of sexual abuse with physical abuse or neglect commenced early in the child's life, have particularly adverse outcomes.

Il s'agit dans cet article d'une étude de suivi: on rend compte de l'état de la santé mentale et de l'ajustement psychosexual à l'âge adulte de 57 jeunes filles qui furent, durant leur enfance, retirées de leur foyer par des agences de Services sociaux à cause d'abus, de négligence ou de brisure familiale. Vingt d'entre elles furent victimes d'abus sexuels au sein de la famille avant l'âge de 14 ans. L'analyse de régression multiple montre que l'abus sexuel en bas âge explique davantage la variance de l'ajustment à l'âge adulte que d'autres facteurs comme la brutalité physique, la négligence ou la séparation maternelle. Tout en tenant compte des limites de l'échantillon, on en arrive à la conclusion que l'abus sexuel au sein de la famille durant l'enfance comporte à long terme des séquelles pour la santé mentale durant l'âge adulte à moins que soit offerte une intervention thérapeutique appropriée. Les déficiences de l'ajustement ne sont pas fonction du fait de retirer l'enfant de son foyer; toutefois si l'abus sexuel est combiné à la brutalité physique ou à la négligence durant l'enfance, on peut s'attendre à des conséquences regrettables.
