The Prevalence and Mental Health Sequels of Child Sexual Abuse in a Community Sample of Women Aged 18 to 27

Abstract

A community mental health survey of 750 women aged 18 to 27 in a large urban centre established mental health profiles using a variety of measures, including investigations of any history of child abuse (including sexual abuse, defined as the unwanted fondling of the child's genital area, or attempted or achieved penetration of the child's body before the age of 17). Overall, 32% of respondents recalled abuse of this type. Of the 750 women, 6.8% had experienced sexual abuse which went on for more than one week. This long-term category included virtually all of those who experienced abuse by a trusted, authority figure. The Trauma Symptom Checklist (TSC) was found to be the most useful instrument among the several used in identifying sexual abuse histories. Scores of 30 or more on the TSC identified 72% of victims of long-term abuse, while 23% of those with scores of 30 + had experienced short-term sexual abuse. Seven percent of individuals with 30 + scores had never experienced (or did not recall) sexual abuse. No particular sub-scale of the TSC had better utility in identifying former victims than did the scale total.

Une enquête de santé mentale communautaire réalisée auprès de 750 femmes agées de 18 à 27 ans, dans un grand centre urbain, a permis d'établir des profils de santé mentale à partir d'une variété de mesures, en plus de relever toute histoire d'abus dans l'enfance (incluant l'abus sexual, defini par toute caresse non voulue des parties génitales de l'enfant, ou bien d'essais ou d'actes de pénétration du corps de l'enfant avant que la personne n'ait 17 ans). Trente-deux pourcent de l'ensemble des répondantes se souvenaient d'abus de ce genre. Parmi les 750 femmes, 6,8% avait subis des abus sexuels pendant plus d'une semaine. L'Inventaire de symptômes traumatiques (Trauma Symptom Checklist [TSC]) s'est révélé, parmi plusieurs instruments utilisés, le plus utile pour identifier les histoires d'abus sexuels. Des scores de 30 ou plus sur le TSC ont permis d'identifier 72% des victimes d'abus sexuels de longue durée, alors que 23% de celles obtenant 30 + avaient subi un abus sexuel de courte durée. Sept pourcent des individus ayant un score de 30 + n'avaient subi aucune expérience (ou ne s'en rappellaient pas) d'abus sexuel. Aucune sous-échelle du TSC n'a eu une plus grande efficacité que l'échelle globale pour identifier les victimes passées.
