Mapping Community Capacity: Identifying Existing Community Assets for Supporting People with Mental Health Problems who Have Been Involved with the Criminal Justice System

Abstract
Continuity of care is critical for people with mental health problems following a period of incarceration or forensic hospitalization––for both mental health recovery and preventing criminal recidivism. Little is known, however, about existing supports and services for the unique needs of this group of people. This case study engaged 20 community stakeholders in Halifax, Nova Scotia, in a capacity-mapping forum. In small focus groups, participants worked through a series of guided capacity-mapping activities. The findings map existing assets according to their policy mandate for providing mental health and/or criminal justice services. Critical challenges to ensuring continuity of care were described.

La continuité des soins pour les personnes ayant des problèmes de santé mentale après une période d'incarcération ou d'hospitalisation médico-légale est essentielle à la fois pour le rétablissement de la santé mentale et pour prévenir la récidive criminelle. Cependant, on ne sait que peu concernant les appuis et les services existants pour desservir cette population. Cette étude comprend 20 participants et participantes provenant d'organismes situés à Halifax en Nouvelle-Écosse qui ont collaboré à un forum d'inventaire des capacités communautaires. En petits groupes de discussion dirigés, les participants et participantes ont travaillé par le biais d'une série d'activités guidées afin de faire l'inventaire des capacités. Les résultats font l'inventaire des atouts existants en fonction de leur mandat d'assurer des soins en santé mentale, des services de justice pénale ou tous les deux. Des défis cruciaux pour assurer la continuité des soins sont décrits.
