Accès au programme d’intervention comportementale intensive (ICI) pour les jeunes enfants autistes au Québec et inégalités sociales de santé

Résumé

La forte demande d’intervention comportementale intensive (ICI) en autisme, s’associant à des ressources publiques limitées au Québec, donne lieu à une sélection des bénéficiaires de services. Nous abordons dans cet article les enjeux d’une justice sociale en analysant la durée d’attente de service et l’accès au programme pour 715 demandeurs de service âgés de 0 à 5 ans. Les résultats démontrent qu’il y a un lien significatif entre, d’une part, l’âge de l’enfant au moment de la demande, la source de revenu/langue d’usage des parents, ainsi que la durée d’attente de services, et, d’autre part, entre l’âge de l’enfant et l’accès aux services. Les enfants plus âgés sont proportionnellement plus nombreux à attendre longtemps avant d’accéder aux services (p < 0,0001), et aussi plus nombreux à ne pas accéder aux services (p < 0,0001). Les enfants dont les parents n’ont pas de revenu d’emploi (p = 0,010) ou possèdent une langue d’usage autre que le français (p = 0,006) débutent plus rapidement les services. Ces résultats orientent vers l’implication des déterminants sociaux dans l’accès sélectif aux programmes d’ICI.

Abstract

The strong demand for intensive behavioral intervention (IBI) in autism, combined with the limited public resources in Quebec, gives rise to a selection of service recipients. We addressed the issues of social justice by analyzing the service waiting time and the access to the early intensive behavioural program for 715 service seekers aged 0 to 5 years. The results show that there is a significant link between, on the one hand, the age of the child at the time of the request, the source of income/parents’ language of use and the waiting time for services; and on the other hand, between the age of the child and the services access. Older children are more likely to wait longer to access services (p <0.0001), and also more likely to not access services (p <0.0001). Children whose parents either do not have an employment income (p=0.010) or have a language of use other than French (∏=0.006) start services more quickly. These results point to the involvement of social determinants in selective access to IBI programs.
