Barriers to Treating Depression in the Family Physician's Office

Abstract

This qualitative research aims to understand, from the standpoint of the family physician, the barriers to treating depression in the office setting. Three primary barriers to treating depression in the family physician's office were identified: systemic, physician-related, and patient-related. The systemic barriers involved the shortage of qualified, publicly-funded counsellors, lack of locally available counselling, and the cost of medication. Physician-related barriers included lack of time and expertise, and inadequacies of the reimbursement system. Patient-related barriers were rooted in the stigma attached to depression and failure to comply with treatment.

Résumé

Cette recherche qualitative vise à comprendre, du point du vue de médicin de famille, les obstacles au traitement de la dépression en cabinet. On identifie 3 principaux types d'obstacles au traitement de la dépression par le médicin de famille en cabinet : les obstacles liés au système, liés au médecin et liés au patient ou patiente. Les obstacles liés au système ont trait au nombre insuffisant de consieillères et conseillers qualifiés dont les services sont payés par l'État, au manque de counseling dans un lieu particulier et au coût des médicaments. Les obstacles liés au médicin ont surtout rapport au manque de temps ou d'expertise, ou encore aux lacunes du système de remboursement. Les obstacles liés au patient ou patiente résultent plutôt des préjugés associés à la dépression et du défaut de se conformer au traitement.
